Zevra Therapeutics Reports Strong Q4 and 2025 Performance with Profit Shift

Zevra Therapeutics (NASDAQ: ZVRA) has announced robust financial results for the fourth quarter and full year of 2025, marking a significant shift toward profitability. The company reported a net revenue of $106.5 million for the year, largely driven by its Niemann-Pick disease type C (NPC) therapy, MIPLYFFA, which generated $87.4 million during its first full year of commercialization. In the fourth quarter alone, total net revenue reached $34.1 million, with $26.4 million stemming from MIPLYFFA sales.

Neil McFarlane, President and CEO of Zevra Therapeutics, characterized 2025 as “a banner year” for the company, emphasizing the growing commercial traction of MIPLYFFA. Chief Commercial Officer Josh Shafer highlighted the positive adherence rates among patients starting therapy, noting that refill rates have been promising, although it is still early to provide comprehensive data on long-term persistence.

Zevra has implemented a “three-pronged approach” to enhance disease awareness and improve patient access to MIPLYFFA. This strategy includes the campaign “Learn NPC: Read between the signs,” an AI-driven targeting model, and partnerships with genetic testing providers. Shafer indicated that market access coverage for MIPLYFFA now reaches 68% of total covered lives in the U.S., with high reimbursement rates established for remaining patients.

International Expansion and Regulatory Progress

Beyond the U.S., Zevra is actively expanding access to MIPLYFFA through its global Expanded Access Program (EAP). By the end of 2025, 113 patients were enrolled in the program. McFarlane noted the company’s growing reputation in Europe, mentioning a new distribution agreement aimed at extending the EAP into select territories outside Europe. He cautioned, however, that patient ordering patterns and enrollment rates may fluctuate as the program stabilizes.

In July 2025, Zevra submitted a Marketing Authorisation Application (MAA) to the European Medicines Agency, which included pivotal Phase 2/3 trial data and results from open-label extensions and a pediatric sub-study. By the end of 2025, the company received a list of questions from EU regulators, and management expressed readiness to respond during the following 90-day period. McFarlane highlighted that higher diagnosis rates of NPC in Europe, compared to the U.S., could be attributed to prior approvals and increased physician awareness.

Advancements in Clinical Pipeline

Zevra is also developing celiprolol for vascular Ehlers-Danlos syndrome (vEDS), a rare connective tissue disorder linked to COL3A1 mutations. McFarlane reported that approximately 7,500 individuals have been diagnosed with vEDS in the U.S. The ongoing Phase 3 DiSCOVER study aims to confirm previous clinical benefits observed in earlier studies. As of the end of 2025, Zevra had enrolled 52 out of a planned 150 patients, with ongoing efforts to connect with specialists and enhance diagnosis through a network of genetic testing centers.

The company is also in discussions with the FDA regarding regulatory options to expedite the celiprolol program, with further engagement anticipated later in the year.

Financial Overview and Leadership Changes

For the fourth quarter of 2025, Zevra reported a net income of $83.2 million, translating to $1.40 per basic share and $1.35 per diluted share. This marks a significant turnaround from a net loss of $105.5 million in 2024. Total operating expenses for the year were $90.4 million, which included $12.7 million allocated for research and development and $77.6 million for selling, general, and administrative expenses.

As of December 31, 2025, Zevra held $238.9 million in cash, cash equivalents, and investments, alongside approximately $61.9 million in total debt. During the earnings call, management confirmed its commitment to quarterly evaluations of capital allocation, focusing on U.S. commercial growth, geographical expansion, and the advancement of celiprolol.

Additionally, Zevra announced leadership changes, welcoming newly appointed Chief Financial Officer Justin Renz, who expressed his intent to engage with stakeholders and support the company’s long-term value creation.
