Wave Life Sciences (NASDAQ:WVE) has announced significant upcoming clinical developments in its obesity and RNA editing programs during a recent meeting with Oppenheimer biotech analyst Cheng Li. The company aims to leverage its advanced oligonucleotide chemistry to address pressing medical needs, particularly in obesity treatment and genetic disorders.

Paul Bolno, President and CEO of Wave Life Sciences, emphasized the company’s decade-long efforts to enhance the efficacy and durability of RNA medicines. By integrating advancements in oligonucleotide chemistry with insights from human clinical genetics, Wave aims to target conditions that have been challenging for other companies.

Focus on WVE-007 and Body Composition

A significant portion of the discussion centered on WVE-007, a small interfering RNA (siRNA) designed to inhibit Inhibin βE and consequently reduce Activin E. Bolno noted that several key opinion leaders (KOLs) in obesity and advocacy groups have highlighted the urgent need for solutions that extend beyond mere appetite suppression. He stated that there is a critical focus on improving body composition, specifically by reducing visceral fat while maintaining lean muscle mass.

Bolno contrasted WVE-007 with traditional incretin-based therapies, which he described as having limitations, including potential loss of lean muscle mass and issues with patient tolerability that may affect long-term adherence. He cited data from the UK Biobank, indicating that individuals with a significant reduction in Inhibin βE exhibit favorable metabolic traits, including lower abdominal fat and improved health markers such as triglycerides and LDL cholesterol levels.

The early clinical observations from a three-month follow-up after a single 240 mg dose of WVE-007 have shown promising results. Bolno highlighted that Wave’s messaging is evolving towards “become leaner, not lighter,” underscoring the importance of overall body composition over scale weight alone.

Upcoming INLIGHT Study Results

Wave Life Sciences is poised to release additional data from its INLIGHT study in the coming weeks, including six-month follow-up data for the 240 mg cohort and three-month data for a 400 mg cohort. Bolno explained that these results will provide insights into the dose-response relationship and the duration of Activin E suppression, which are crucial for understanding the potential for sustained fat loss.

He cautioned against expecting linear extrapolations in humans based on incremental biomarker changes, emphasizing that the depth of Activin E suppression is vital. While details regarding baseline characteristics for the 400 mg cohort remain undisclosed, Bolno mentioned that this cohort is expected to include participants with higher body mass indices (BMIs) and comorbidities, addressing a broader patient population.

Wave is also accelerating a Phase 2a segment of the program that will allow for the enrollment of higher-BMI individuals, with plans to initiate this phase in the first half of 2024.

Bolno expressed optimism about the potential to evaluate biomarker effects in participants with comorbidities, such as changes in hemoglobin A1c and lipid levels. He indicated that including MRI-based imaging of liver fat in the upcoming study could further elucidate the effects of WVE-007 on hepatic fat reduction.

The prospect of treating metabolic dysfunction-associated steatohepatitis (MASH) with a once- or twice-yearly subcutaneous therapy was also discussed. Bolno noted that such an approach could yield significant benefits in both obesity management and as a standalone treatment.

On the regulatory front, Bolno anticipates meeting conventional expectations for body weight reduction through a one-year study in an obese population while focusing on a differentiated profile that emphasizes body composition and muscle preservation. He referenced draft regulatory guidelines that reiterate a 5% threshold for weight loss, highlighting the importance of incorporating body composition metrics into product labeling.

Commercial interest in WVE-007 is reportedly robust, with potential applications as a monotherapy, in combination with incretins, and for long-term maintenance therapy. Bolno pointed out that industry trends toward reduced starting material costs and enhanced synthesis yields support the feasibility of large-scale production.

Additionally, Bolno addressed the company’s RNA editing program targeting alpha-1 antitrypsin deficiency (AATD). Upcoming data from a 400 mg multiple-ascending-dose cohort is expected to provide further insights into the therapeutic potential of this program. Wave aims to help patients achieve a heterozygous-like phenotype, enabling protective protein responses during acute exacerbations.

Bolno cited previous findings from a 200 mg single-dose cohort, indicating that patients could mount significant responses two weeks post-dosing. He anticipates receiving regulatory feedback by mid-2024, exploring possible accelerated approval pathways based on biomarker strategies.
